If your start date is less than one week from today, please call the office to enroll by phone. If not, use this form and our office staff will contact you after we have received your submission. A deposit of $50 per person is required to confirm all lessons. You will be automatically re-directed to Paypal after you complete the form below. You do not have to have a Paypal account as all major credit cards are accepted.
